You will manage patient care coordination and clinical documentation in a remote capacity.

Responsibilities

• Execute case management protocols for assigned patients.
• Maintain accurate patient charts and clinical records.
• Coordinate home care services and patient support.
• Utilize digital tools for clinical documentation and communication.

Required Skills

• Registered Nurse (RN) licensure.
• 2+ years of professional nursing experience.
• Direct experience in Case Management.
• Proficiency in Charting and clinical documentation.
• Experience in Home Care environments.
• Strong computer literacy for healthcare systems.

Preferred Skills

• Ability to work independently in a remote setting.
• Experience with electronic health record (EHR) systems.
