You will lead the coordination of a multidisciplinary team to deliver holistic, person-centric care management to a diverse health plan population.
Responsibilities
Lead a multidisciplinary team including social workers, dietitians, pharmacists, and medical directors to meet member needs via telephonic and digital channels.
Assess member health, psychosocial needs, and cultural preferences to develop, implement, and monitor individualized care plans.
Coordinate care delivery and resources such as community services, mental health support, and substance abuse services.
Educate members and caregivers on health literacy, self-management, medication plans, and nutrition.
Document all member interactions accurately and manage transitions of care, including facility discharges and transfers.
Required Skills
Current, active, and unrestricted Michigan Registered Nurse license.
Nursing Diploma or Associate Degree in Nursing.
3 years of clinical nursing experience in clinical, acute/post-acute care, or community settings.
Ability to obtain Certified Case Manager (CCM) certification within 18 months of hire.
Proficiency with Microsoft Office tools including OneNote, Outlook, Teams, and Word.
Experience managing patients through telephonic and digital channels.
Preferred Skills
Bachelor’s degree in nursing.
1 year of case management experience in a managed care setting.
