Description

You will lead complex project management initiatives within a Care Case Management environment, focusing on application and data migration.

This role is on-site.

Responsibilities

• Lead and manage application and data migration projects within a Care Case Management environment.
• Identify requirements, perform gap analysis, and manage end-user communication.
• Collaborate with stakeholders including case managers, families, support providers, and other end users to ensure project success.
• Facilitate meetings, testing, and test preparation.
• Create and maintain project schedules, status reports, and meeting agendas.

Required Skills

• 5+ years of technical project management background in Care Case Management environments (social work, health & human services, hospital settings).
• Experience managing application and data migration projects.
• Experience working with HIPAA-constrained data elements and electronic health records (EHR), electronic medical records (EMR), and patient case management.
• Exceptional verbal and written communication skills.
• Strong analytical, judgment, and decision-making abilities.
• Proficiency with PowerPoint and Excel.
• Experience with technology-driven change management.
• Ability to work and team with people across functions, sectors, and locations.
