Description

You will support patients in regaining optimum health and improving functional capability within their specific settings.

Responsibilities

• Conduct patient assessments and develop comprehensive treatment plans, including necessary referrals.
• Maintain accurate patient medical records and documentation via the Health Point system.
• Ensure all medical documentation complies with state regulations and Doc Go standards.
• Collect data and report on program performance using strong organizational methods.

Required Skills

• Valid Licensed Master of Social Work (LMSW) license issued by the NYSED.
• Primary Source Verification of licensure is required.
• Minimum of 1 year of experience as an LMSW.
• Proficiency in English (speaking, reading, and writing).
• Strong organizational skills for data collection and performance reporting.
• Experience with medical documentation and patient record maintenance.
