Description

You will manage care coordination for members in residential facilities and private homes across Sarasota County.

You will own the release pipeline

Responsibilities

• Conduct member-facing visits in homes or care facilities for approximately 80% of your workload.
• Coordinate long-term care requirements and services for assigned members.
• Execute discharge planning to ensure continuity of care.
• Manage care planning and member oversight throughout the contract period.

Required Skills

• 5+ years of experience in healthcare coordination or related roles.
• Proven expertise in Care Management.
• Direct experience with Discharge Planning.
• Demonstrated knowledge of Long-term Care protocols.
• Ability to travel to member locations within Sarasota County.
• Bachelor's degree or equivalent graduate-level education.
