Description

You will manage care coordination and member support for the Orange and Osceola areas, with 80% of time spent on member-facing field work.

Responsibilities

• Conduct member-facing visits in residential homes and care facilities within Region 7 counties.
• Manage discharge planning processes for assigned members.
• Coordinate long-term care services to meet specific member needs.
• Travel locally within the Orange and Osceola regions as required.

Required Skills

• 5+ years of experience in healthcare coordination or similar roles.
• Proven expertise in Care Management.
• Direct experience with Discharge Planning.
• Specialized knowledge in Long-term Care protocols.
• Ability to perform 80% member-facing field work.
• Graduate-level education in any field.
