Manage patient care coordination and HEDIS activities for clients in Illinois.

This role is remote.

Responsibilities

• Coordinate care management tasks and track patient progress.
• Schedule and manage patient appointments efficiently.
• Execute HEDIS-related activities to ensure compliance.
• Maintain professional phone etiquette during all patient and provider interactions.
• Navigate Medicaid requirements and processes accurately.

Required Skills

• Valid Licensed Practical Nurse (LPN) credentials.
• 5+ years of professional nursing or care management experience.
• Proficiency in HEDIS protocols and data collection.
• Direct experience with appointment scheduling systems.
• Strong knowledge of Medicaid systems and regulations.
• Ability to work remotely from Illinois.

Preferred Skills

• Proven ability to maintain high standards of phone etiquette.
