Description

Lead multidisciplinary team coordination to deliver person-centric care management for a diverse health plan population.

Responsibilities

• Lead the coordination of multidisciplinary teams to deliver holistic care management programs.
• Serve as the primary point of contact for members, caregivers, and providers via phone, email, text, and online messaging.
• Assess, develop, implement, monitor, and evaluate care plans to optimize member health across the care continuum.
• Partner with members, providers, and community resources to implement care plans and achieve clinical goals.

Required Skills

• Current, active, and unrestricted Michigan Registered Nurse license.
• 3 years of clinical nursing experience in clinical, acute/post-acute, or community settings.
• Experience with Case Management, Managed Care, and Utilization Review.
• Knowledge of Acute Care and Medicare.
• Nursing Diploma or Associate degree in nursing.
• CCM certification required or to be obtained within 18 months of hire.

Preferred Skills

• Bachelor of Science in Nursing (BSN).
• 1 year of case management experience in a managed care setting.
• Experience managing patients via digital channels, mobile applications, and messaging.
