Description

You will provide clinical social support and navigation for patients within a primary care setting.

Responsibilities

• Navigate social determinants of health (SDOH) including food and housing insecurity.
• Conduct in-person or remote social needs screenings and assessments for referred patients.
• Coordinate access to social services such as mental health, housing, clothing, and food resources.
• Compile and maintain a resource directory detailing eligibility criteria, referral processes, and contact information.
• Collaborate with the primary care team to execute patient care goals.

Required Skills

• 5+ years of experience in social work or a related clinical field.
• Proficiency in Psychotherapy.
• Experience developing and managing Care Plans.
• Expertise in Discharge Planning.
• Background in Mental Health Counseling.
• Ability to manage a regular 8:00 AM to 4:30 PM shift.

Education

• Bachelor's degree or higher (Any Graduate).
