Description

You will provide clinical social support and resource navigation for patients within a primary care setting.

This role is on-site.

Responsibilities

• Navigate social determinants of health (SDOH) by addressing food insecurity, housing instability, and other social needs.
• Conduct in-person or remote social needs screenings and assessments for referred patients.
• Coordinate access to social services, including housing, clothing, food, and mental health resources.
• Manage patient assistance programs, including applications for CoverRx, RxOutreach, and low-cost prescription options.
• Maintain updated resource lists for SDOH and specialty referrals, including eligibility and contact details.

Required Skills

• 5+ years of experience in social work, social services, or public health.
• Experience in Case Management and Care Plans.
• Knowledge of Primary Care workflows.
• Experience with Psychotherapy.
• Ability to serve as a patient-provider liaison with pharmaceutical companies.
• Graduate-level education.
