Description

You will provide clinical social work support within a primary care setting to address social determinants of health and facilitate patient care coordination.

Responsibilities

• Navigate social support for determinants of health including food and housing insecurity.
• Conduct patient social needs screenings and assessments in person or remotely.
• Manage case activities including hospital discharge planning, follow-up, and medical equipment procurement.
• Coordinate specialty and imaging referrals by scheduling, tracking, and obtaining medical records.
• Assist patients in accessing low-cost prescription options and patient assistance programs.
• Document all interventions in patient records and update clinical staff as necessary.

Required Skills

• Bachelor’s Degree in a relevant field.
• 7+ years of professional experience.
• Experience with social determinants of health (SDOH) navigation.
• Proficiency in case management and discharge planning.
• Ability to coordinate with hospitals, specialists, and community partners.
• Knowledge of prescription assistance programs such as CoverRx.
• Competency with personal computers and standard office equipment.
• Access to a personal vehicle for required tasks.
