Description

Discharge planner for skilled nursing and acute rehab populations. Own the transition from facility to home or alternative care.

Responsibilities

• Develop social care plans and conduct resident assessments for safe patient discharge.
• Coordinate with RN case managers and interdisciplinary teams to facilitate patient transitions.
• Manage administrative tasks including medical forms, charting, reports, and transportation arrangements.
• Advise patients and families on Medicare, Medicaid, and financial assistance programs.
• Consult staff to resolve psychosocial concerns and manage grievance tracking.

Required Skills

• LMSW licensure.
• Minimum 2 years of recent hospital experience.
• 3+ years of professional experience.
• Knowledge of rules and regulations governing SNU and ARU settings.
• Ability to interpret data and make decisions based on age-specific growth and development.
• Proficiency in discharge planning and psychosocial assessment.
• Strict adherence to HIPAA regulations and departmental confidentiality.
